Position:  Patient Care Manager – Social Work, Discharge Planning, and Patient Flow

Department: Social Work & Patient Flow

Status:  Full-time

Hours:  Monday to Friday days (must be available for early morning and late evening meetings) Hours subject to change based on operational requirements.

Reporting to the Director of Rehab, Geriatrics, and Patient Flow, the Patient Care Manager (PCM) is an inspiring leader who creates a work environment that promotes collaborative teamwork and an exemplary patient and family experience. She/he is responsible for the coordination and integration of clinical services and supporting roles to optimize patient access and patient flow. She/he will coordinate and facilitate the seamless flow of internal and external patient admissions, transfers, and discharges that ensure “the right patient is placed in the right bed at the right time”, and maximize the utilization and allocation of patient beds and clinical resources. PCM responsibilities also include planning, implementing and evaluating activities related to the provision of high quality patient & family centred care services and programs; selecting, hiring and developing staff; working collaboratively with the team; and maintaining fiscal accountability. 

This is what you'll be doing:

  • Coordinate and redeploy staff to achieve appropriate skill mix based on patient safety, departmental coverage needs, and associated risks
  • Expedite, prioritize, synchronize, and mediate clinical issues and barriers to review and respond to patient care delays; track, prioritize, and facilitate key discharge related workflow activities (e.g. conditional discharges, ancillary (physiotherapy, occupational therapy) and physician consults)
  • Collaborate with inpatient managers to identify process improvement strategies to improve the current inpatient system and reduce patient length of stay (LOS)
  • Define and prepare reports to inform management, physician chiefs, and administrative team of patient activity and bed utilization to manage surges in patient volumes
  • Identify and communicate clinical concerns or system issues that occurred during evening or weekends to manager for action
  • Manage staff performance within portfolio
  • Coaching/influencing, leading staff as to expected Halton Healthcare values
  • Promote strong linkages and relationships with community partners and programs (e.g. LHIN).
  • Monitor and ongoing improvement of key performance indicators.
  • Develops and manages cost centre budgets in accordance with Broader Public Sector Guidelines; is accountable for an annual balanced budget
  • Evaluate and optimize patient satisfaction of discharge planning functions.
  • Develop, review, and revise evidence-based policies, procedures and guidelines related to discharge planning, social work, and patient flow.

  • Undergraduate degree in a healthcare related field (Masters prepared or evidence of substantial completion preferred)
  • Current member and in good standing with their professional college (Social Work preferred)
  • A minimum of 3 years of recent experience as a Manager in a similar hospital environment
  • Proven leadership skills
