Hours: Various rotating shifts:  Mon-Thurs:  0800 a.m. to 8:00 p.m./ Fri & Sat - Days (Hours are subject to change based on operational needs)

Position Overview:

Halton Healthcare Mental Health Program is recruiting a Nurse Practitioner (NP) for our newly developing Practical Clinic. An NP is a registered nurse with additional educational preparation and experience who possess and demonstrate the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform specific procedures within his/her legislated scope of practice. The NP role is derived from blending clinical diagnostic and therapeutic knowledge, skills and abilities within a nursing framework that emphasizes holism, health promotion and partnership with individuals and families, as well as communities. Nurse Practitioners have the knowledge to assess population health trends and patterns and to design services that promote healthy living. They provide leadership in the development, implementation and evaluation of strategies to promote health and prevent illness and injury, and they work with inter-professional teams, other health-care providers and sectors and community members. Nurse Practitioners collaborate in the development of policy to influence health services and healthy public policy.

This is what you will be doing:

The Practical Clinic NP utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for clients who are referred to the clinic by a general practitioner and are requiring basic prescribing of first round treatments for mood and anxiety disorders in an outpatient setting in collaboration with the Psychiatrist lead of the clinic. The NP will rotate between providing care to clients within a group based medication visit (GMV) and liaising on care planning with primary care providers. The NP will be expected to provide a comprehensive health assessment and diagnosis of mood and anxiety based illnesses, develop and implement a plan of care, provide health counseling in a group based environment to clients and their family members and health promotion. Furthermore, the NP will assist the interdisciplinary team in implementing a plan of care that will enable individuals to manage their mental health care and avoid unplanned visits to the Emergency Department.

Additional Responsibilities may include:

Direct Comprehensive Care: Obtains and documents a comprehensive health history, performs a physical examination on patients requiring assessment, collaborates closely with the Psychiatrist and primary care referral source and other members of the interdisciplinary team and coordinates care involving other healthcare providers.

Leadership: Provides leadership in the development, implementation and monitoring and evaluation of quality management programs. Participates and provides leadership in the strategic planning process for the delivery of care. Supports educational initiatives directed at nursing and allied health to promote and provide medical care that meets best practice standards.

Education: Contributes to the development, implementation and evaluation of patient education programs. Conducts educational teaching sessions with patients and/or families with a focus on health promotion, disease prevention, and impact of illness experience on the patient and family.

Research: Encourages, supports, and facilitates the development of researchable questions in clinical practice.

Consultation: Provides consultation internally and externally for clinical practice issues within the NP’s area of specialty and scope of practice.

  • Current registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario required
  • Completion of a Master's Degree in Nursing required
  • CNA Specialty Certification in Mental Health preferred.
  • Minimum of 5 years recent nursing experience in a Mental Health setting required
  • Direct experience in Mental Health preferred
  • Experience and interest in group-based facilitation techniques
  • Innovative, creative and professional role model
  • Demonstrated ability to prioritize/organize multiple demands
  • Ability to work autonomously,
  • Proficient with Microsoft Office applications including Outlook, Word, Excel, PowerPoint
  • Ability to meet the physical demands of the position,
  • Satisfactory employment and attendance record required

Salary: $49.15 to $60.11 per hour
