ABOUT DIRECTIONS YOUTH SERVICES

Directions Youth Services is a collection of services and locations supporting vulnerable youth. This includes a 24-hour drop-in centre, outreach teams, pre-employment programs, two safe houses, and a youth detox centre. The trauma-informed and client centred philosophy creates safety for youth to access a full spectrum of supports including primary care, mental health and substance use resources. Directions is a division of Family Services of Greater Vancouver (FSGV), which for 90 years has provided trauma counselling and essential resources for children, families and individuals facing life’s challenges. HERE’S WHAT WE CAN DO TOGETHER 0.8 of this position provide case management service and clinical and consultative social work services to 12 shelter clients and centre clients experiencing brittle support systems, homelessness, social functioning and mood and behaviour issues. Duties include conducting social and psychosocial assessments, providing individual, family and group counselling services, maintaining client records, attending interdisciplinary clinical/case conferences, and providing home and community care assessments. Identify applicable community resources for clients, contribute to the effectiveness and development of community resources and planning with clients for care and support in accordance with their health needs. Within a multidisciplinary team context, provide continuity of care across the continuum by liaising with team members, developing/evaluating care plans and coordinating, implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care and supporting transition. 0.2 of this position provide clinical supervision, mentorship, and psycho-social group education for skill development and self-care to hourly youth support workers without extended health benefits, also providing short term solution focused support, referral, and clinical response to workplace trauma. YOUR KEY CONTRIBUTIONS Receive, prioritize referrals and screen clients for eligibility of services by conducting psychosocial assessments, interviewing the client obtaining relevant information, gathering social data regarding the client's support system, living situation, financial status, and current stressors/concerns to determine the level of support required. Interpret the data lo therapeutically impact the clients coping abilities and anticipated needs to maximize client strengths and self-determination with a focus on self - management. Provide individual and/or group counselling mediation, conflict resolution services and crisis interventions by method s such as exploring emotional responses to illness/injury in order to identify problems, personal concerns and treatment objectives, and to facilitate change in coping style, behaviour, attitude, feelings and adaptive responses to an adverse life event. Coordinate care for clients by applying case management principles, carrying out health, psychosocial, functional/environmental and safety/ risk assessment, identifying problems, addressing issues or collaboration with clients for variances from the plan of care, work with the appropriate clinicians and interdisciplinary team member and/or encouragement for clients to identify and utilize community supports and agencies. Facilitate and implement the teaching of and collaboration with staff regarding best practice strategies focusing on strengths, illness prevention, health maintenance and health promotion by supporting learning needs, development and implementation of health teaching strategies, maintenance of current reference material for specialized staff groups and evaluation of the outcome of learning.
